Supported Housingwith Central City Health

Central City Health recognizes the importance of an individual having safe housing to overall health and well-being. CCH’s commitment to integrating health services starts with our proactivity in combating homelessness. The basic provision of shelter is bedrock to individuals and families and their ability to care for themselves. CCH staff works to provide new solutions, strategies, and approaches to house our consumers. CCH also offers comprehensive services to better equip our consumers to maintain housing.  

The Supportive Housing Division is comprised of numerous programs servicing individuals with varying needs and/or circumstances including people with severe mental illness and co-occurring disorders (severe mental illness and substance abuse) and people with physical or health disabilities who may also have a substance abuse problem. The housing department staff is specially trained to assist people in the transition from homelessness to permanent housing, enabling them to live as independently as possible.

CCH serves as the Lead Service Provider to seven PBV Detroit developments and as such, processes applications and provides voluntary, on-site supportive services to 172 households. The goals of the supportive services are to keep families and individuals housed, address family and tenancy issues and promote self-sufficiency. Last year CCH processed more than 400 applications for PBV housing. 

Project Based Voucher Program (PBV)

The Project Based Voucher (PBV) program is designed to provide affordable housing in properties funded through the Low-Income Housing Tax Credit (LIHTC) Program, to individuals that fall under the special needs category, which can include individuals with the following conditions: 

  • Homelessness 
  • Survivors of Domestic Violence 
  • Youth Aging out of Foster Care 
  • Mental Illness 
  • Physical Disabilities 
  • HIV AIDS 

 Project Based Voucher (PBV) applications can be completed on Mondays and Wednesdays from 9 a.m. to 3 p.m. at the office.

Your Care Team Will Work Hard To:

  • Listen to your health concerns
  • Answer your questions
  • Provide you with a team of professionals that will focus on all aspects of your health and wellbeing
  • Provide easy access to services by offering extended office hours and after hours consultation by phone
  • Communicate test and treatment results promptly
  • Provide reminders for follow up and preventative care
  • Coordinate your care with other health care providers on site and through community referrals
  • Provide you with the treatment and guidance you need to care for yourself. 

We Ask That You:

  • Provide a complete medical history, including information about care received by outside providers
  • Get to know your health care team and help them get to know you. Don’t be afraid to ask questions!
  • Work with your care team to set your own treatment goals to improve your health
  • Make your best effort to attend each appointment or call us at least 24 hours before your visit if you need to reschedule
  • Share your medical history by asking any other providers that you see/have seen, to send their reports to your PCP
  • Bring any medications, vitamins or supplements you are taking to your appointments
  • Bring a list of questions for your care team so that we can coordinate your care together!
