Employment Services

CCH brings hope to the chronically unemployed by bridging the gap with employers through its evidence-based practice program. Employment is an important means of independence and self-worth, but for many it is out of their reach. Candidates participating in CCH’s IPS Supported Employment Services come from Wayne County and may be dealing with chronic health conditions, homelessness and/or mental health or substance use disorder issues. CCH’s IPS Supported Employment staff provide participants with services and support necessary to obtain and maintain competitive employment in the community. The intent of the program is to ensure that adults with a severe mental illness achieve and maintain desired community-based employment. The employment pays at least minimum wage, while performing the same work alongside others without psychiatric disabilities. This increases recovery and self-sufficiency, and decreases dependence on public systems of care.

During 2024, 62 individuals were referred to IPS Supported Employment Services through various other CCH programs. Of these individuals, CCH found competitive employment for 20 of them, with an average hourly wage earned of $14.34.

Central City Health provides résumé building services, works directly with MRS (Michigan Rehabilitation Services), and provides direct referrals for our members as well as additional training and work-ready services (uniforms, work boots, etc.). CCH employment specialists work with employers to get members hired and to keep them on the job. CCH also assists with transportation to and from jobs, expense budgeting, and benefits planning (social security/SSI). ISP employment specialists work directly with the member and their integrated care team to alleviate issues with their health that may affect their performance on the job (i.e. medications).  We also work with members to schedule medical appointments so it doesn’t impact attendance on the job. CCH’s team works with members to help secure the job they want, and we have a zero exclusion policy, including people struggling with mental health, homelessness and other struggles.

Insurance Assistance

At CCH, all patients have equal access to care, regardless of their ability to pay for services.  If you do not have health insurance or have questions about your existing plan coverage, we have staff available to help.  There are staff available at all locations to help you obtain or understand your health plan coverage.

 We also offer all services at a sliding fee discounted rate, based on household size and income, for those who are uninsured or underinsured.  We want all patients to have access to the care they need, when they need it.

Your Care Team Will Work Hard To:

  • Listen to your health concerns
  • Answer your questions
  • Provide you with a team of professionals that will focus on all aspects of your health and wellbeing
  • Provide easy access to services by offering extended office hours and after hours consultation by phone
  • Communicate test and treatment results promptly
  • Provide reminders for follow up and preventative care
  • Coordinate your care with other health care providers on site and through community referrals
  • Provide you with the treatment and guidance you need to care for yourself. 

We Ask That You:

  • Provide a complete medical history, including information about care received by outside providers
  • Get to know your health care team and help them get to know you. Don’t be afraid to ask questions!
  • Work with your care team to set your own treatment goals to improve your health
  • Make your best effort to attend each appointment or call us at least 24 hours before your visit if you need to reschedule
  • Share your medical history by asking any other providers that you see/have seen, to send their reports to your PCP
  • Bring any medications, vitamins or supplements you are taking to your appointments
  • Bring a list of questions for your care team so that we can coordinate your care together!
