Outpatient Services

CCH provides behavioral health treatment tailored to meet the needs of each person.  We offer high-quality care that is sensitive to your expressed needs, desires, culture and belief systems, including:  

  • Psychiatric Evaluation and Medication Management

The Clinic Services Program employs recovery-based principals to improve patient understanding of their own abilities and encourage an active life, personal independence and a positive sense of self regardless of the level or degree of their mental health condition.  Through our range of programs, we give people the tools they need to maintain a meaningful life, despite physical or behavioral health challenges. 

Substance Abuse Services

If you or a friend or loved one is struggling with addiction, there are resources available to you.  CCH offers specialized services designed to help with addiction issues.  A substance use disorder can lead to: 

  • Using more substance than originally intended or planned  
  • Inability to control or reduce or decrease current substance use 
  • Failure to fulfill personal responsibilities at home, work, or school 
  • Constant thoughts and/or obsessions centered around “craving” the substance 
  • Continuing to use substances despite any health problems 
  • Damaged relationships with friends, family members, colleagues, etc.    
  • Giving up or reducing activities you enjoy due to isolation from substance use 
  • Built up tolerance of needing larger amounts of the substance to get the same desired effect 
  • Experiencing withdrawal symptoms after stopping use of the substance  
  • Separation from others causing loneliness and isolation  

CCH believes in the basic principles of recovery and providing an open door.  Our integrated service model addresses all aspects of a person’s life and provides the hope and resources needed to make positive changes toward recovery. 

Co-Occurring Disorders

Staff are specially trained to identify and treat individuals experiencing mental illness and substance use, also known as co-occurring disorders.  The Integrated Dual Disorder Treatment (IDDT) program promotes sobriety, symptom management, and an increase in independent living for all members. The IDDT program utilizes a Person-Centered and Individualized Treatment tailored to each person’s unique circumstances and life stage. Furthermore, the IDDT team uses a combination of psychological, educational, and social interventions to help our members define and achieve their goals, which can include stable housing and sustainable income. The team also promotes a positive atmosphere and encourages others to find their own unique pathway to recovery.   

Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is a service-delivery model that provides comprehensive, locally based treatment to people. Unlike other community-based programs, ACT provides highly individualized services directly to members.  ACT recipients receive the multidisciplinary, round-the-clock staffing of a treatment team within the comfort of their own home and community. To have the competencies and skills to meet a client's multiple treatment, rehabilitation, and support needs, ACT team members are trained in the areas of psychiatry, social work, nursing, substance abuse, and vocational rehabilitation.  

Adult Foster Care (AFC)

Adult Foster Care (AFC) homes are residential settings that provide 24-hour personal care, protection, and supervision.  Individuals in adult foster care settings can developmental disability and/or a mental illness and could be physically challenged or are in some way unable to live without direct supports.  Residents receive an additional range of necessary services, supports, or treatment provided in an adult foster care facility to meet the unique daily living challenges of individuals with a serious mental illness or a developmental disability. 

Youth & Children’s Outpatient Mental Health Services

If your child, or a young person in your life, is struggling with emotional, behavioral, or mental health challenges, CCH is here to help. We offer outpatient mental health services for children and youth ages 6 to 21, providing specialized care to support their emotional well-being and development.Mental health challenges in young people may show up as:

  • Sudden changes in mood, behavior, or school performance
  • Difficulty concentrating or staying focused
  • Withdrawing from family, friends, or favorite activities
  • Persistent feelings of sadness, worry, or anger
  • Trouble sleeping or changes in eating habits
  • Risky behavior or difficulty following rules at home or school
  • Expressions of hopelessness, low self-worth, or anxiety
  • Difficulty coping with stress or loss

CCH’s compassionate team includes a child therapist, youth peer support specialist, and case manager to walk alongside families and ensure a strong network of care.We believe in early intervention, family involvement, and the power of connection. Our integrated approach addresses not only the emotional needs of young people but also the environmental, social, and behavioral factors that impact mental health

Your Care Team Will Work Hard To:

  • Listen to your health concerns
  • Answer your questions
  • Provide you with a team of professionals that will focus on all aspects of your health and wellbeing
  • Provide easy access to services by offering extended office hours and after hours consultation by phone
  • Communicate test and treatment results promptly
  • Provide reminders for follow up and preventative care
  • Coordinate your care with other health care providers on site and through community referrals
  • Provide you with the treatment and guidance you need to care for yourself. 

We Ask That You:

  • Provide a complete medical history, including information about care received by outside providers
  • Get to know your health care team and help them get to know you. Don’t be afraid to ask questions!
  • Work with your care team to set your own treatment goals to improve your health
  • Make your best effort to attend each appointment or call us at least 24 hours before your visit if you need to reschedule
  • Share your medical history by asking any other providers that you see/have seen, to send their reports to your PCP
  • Bring any medications, vitamins or supplements you are taking to your appointments
  • Bring a list of questions for your care team so that we can coordinate your care together!
