In over 40 years, Central City Integrated Health has been working to change the paradigm of care in the Detroit community—high-quality care for every person, no matter their station in life.

The health of families and children is impacted by numerous factors. Families moving from homelessness, lack of financial resources, access to food, health literacy, and concentrated poverty can all impact health. CCIH works with you to identify health concerns and interventions that can improve the overall well-being of your family. Both children and parents will be engaged in the health­-centered education with the goal of addressing immediate health concerns as well as social determinants of health.

Complete health care for the entire family including:

  • Health screenings, well-child visits, and physicals
  • Care for diabetes, asthma, blood pressure, and cholesterol
  • OB/GYN, pediatric & adolescent medicine, internal & geriatric medicine, and HIV/AIDS care
  • Quick access to lab and x-ray services (urinalysis, blood glucose, and pregnancy testing)

Primary health care and enabling services that are provided through a nurse-managed program. The health care services are enhanced through case management, behavioral and substance abuse services, health education, outreach and enrollment, and referral arrangements for specialty health care, laboratory services, diagnostic testing, and dental care.

As an integrated Nurse Managed Health Center model, we utilizes nurse practitioners with a collaborating family practice physician to offer prevention, screening, early health care interventions and chronic disease management that will improve the health of Detroit residents, including employees of community businesses and residents of public housing.

What if I haven’t seen a doctor in a long time?

Even if you have not seen a doctor recently, we will evaluate your current condition and work with you to improve your health.

What if I have some major health issues?

Our staff is experienced with every major medical diagnosis. We can refer you to specialists to make sure you receive the care you need.

What if I don’t have insurance? How much does it cost?

We take many insurance plans, including Medicaid. We never turn anyone away based on their ability to pay. We will work with you and provide care regardless of your situation. We practice with a sliding fee scale based on income. A copay may apply depending on your insurance.

Do I need an appointment?

It is best to schedule an appointment to reduce your wait time. However, walk-ins are welcome. We will always let you know about your estimated wait time.
