CCIH is designated as an approved practices service site in a community with limited access to health care by the National Health Service Corps (NHSC). In exchange for committing to service at CCIH, NHSC provides financial support in the form of student loan repayment and scholarships.
Clinical Nurse Practitioner - Psychiatry (F/T and P/T)
Provides advanced clinical nursing care to a selected population of clients. Assumes leadership and provides expertise for intake health history. Collaborates with Psychiatrist to provide psychiatric evaluation, and team members to recommend program placement and coordinate interim treatment planning. Improves clinical staff quality of care through consultation with and education of nursing personnel, clients and their significant others. Serves as preceptor for undergraduate and nursing graduate students.
Staff Nurse (RN)

The Staff Nurse provides care to patients/families, of all ages, in an outpatient setting by assessing, triaging, planning and intervening in actual or potential health problems, including evaluating the patient's response to care. These responsibilities also include patient/family education and health maintenance strategies. The Staff Nurse is a member of a designated health care team and collaborates, coordinates and guides team members in planning patient care and achieving goals. He/She must demonstrate knowledge of the principles of human growth and development over the life span, as well as chronic disease identification and management and interpret data reflective of the patient's status to identify requirements relative to his or her specific needs.
The duties of a dental assistant are among the most comprehensive and varied in the dental office. The dental assistant performs many tasks requiring both interpersonal and technical skills. Working with the clinical staff to ensure patient care and satisfaction are of top priority.
Family Nurse Practitioner
Provide primary care services to patients at DCC Health Center. Within this role the Family Nurse Practitioner is responsible for daily patient office visits, medication prescriptions and prescription refills, appropriate and complete clinical documentation, patient paperwork, clinical record reviews for quality improvement and evaluation of clinical quality activities.
Working under the supervision of the Division Manager, Health Center support staff will conduct operations with maximum efficiency and professionalism in order to promote the mission and philosophy of providing quality health care to the medically underserved. Health Center support staff will perform a variety of clerical tasks where the processing of documents, records, data and other material is a substantial and essential part of the job function. Support staff is required to work within the policies and general office guidelines established by CCIH. They should be acquainted with and have knowledge of offered programs and services and function as a member of the medical home team.
The Outreach Worker reports to Division Manager, and works collaboratively with the Health Educator to connect persons in the underserved community with services provided by the Health Center, provide health education in the community, assist individuals to enroll and maintain participation in health and social services programs both with CCIH and the community. Work is primarily conducted in the community.
The Therapist reports to the Division Manager and clinical functions are under the leadership of the Team Leader. Assumes primary case responsibilities for adult individuals, families and groups; provides consultative services to community organizations and individuals, and performs related duties, as may be requested.
Psychiatrist (F/T and P/T)
Responsible to the Chief Psychiatrist for the provision of specialized psychiatric services which include, but are not limited to: psychiatric evaluations, medication reviews, and consultation on an on-going basis to the Treatment Team in the development and implementation of direct services.

The Clinical Coordinator assists in the development and support of workflow/process improvements, while also providing individual, family and group psychotherapeutic interventions. He/She is responsible for assisting in program development and monitoring, and establishing trusting relationships with patients and their families while providing general support and encouragement.
The Case Manager reports to the Division Manager and clinical functions are under the leadership of the Team Leader. The Case Manager will provide case management services for identified clients being served by the Agency and who are referred through the Mental Health Court. Services are provided in the Agency, the courts, residential settings, and on an Outreach basis.
The Therapist/Team Leader implements the policies and procedures of the Agency under the supervision of the Division Manager and assumes the day-to-day clinical leadership of the respective program staff and/or other programs as assigned. He/She also assumes primary responsibility for the initial assessment, treatment planning and implementation per UNIT guidelines.
The Therapist reports to the Division Manager and clinical functions are under the leadership of the Team Leader. Assumes primary case responsibilities for adult individuals, families and groups; provides consultative services to community organizations and individuals, and performs related duties as may be requested.
Therapist Case Manager (AFC)
The Case Manager will provide case management services for identified clients being serviced by the Agency. Services are provided in the Agency and on an Outreach basis.
The Billing/Coding Specialist works under the direct supervision of the RCM Supervisor and primarily performs duties and tasks relative to billing and reimbursement processes. The Billing/Coding Specialist is responsible for the complete revenue cycle process which includes, reviewing claims prior to submission, submitting claims to the Clearinghouse, following up on the claims and working any rejections and open outstanding claims reports. This position is required to stay abreast of any industry changes and to review and educate front office staff on any necessary changes in the data entry/insurance verification process as it relates to the reimbursement processes.
Housing Supportive Services Facilitator (PBV)
The PBV Housing Supportive Services Facilitator reports to the Housing Director or the Housing Coordinator. The goal is to carry out supportive services for project based housing, as identified in project specific Memorandums of Agreement, by assisting families in maintaining housing that is consistent with their goals. Through assessments and appropriate support services, families are assisted and motivated towards maintaining housing, improving quality of life and increasing family income.
Peer Specialist/Consumer Mentor
A primary consumer or past consumer who is able to provide the supports and information that would benefit a consumer’s ability to reside in the community. The Peer Specialist is someone who has received mental health services and at the time of employment demonstrates a willingness to share his or her knowledge and skills in obtaining recovery with other consumers. Through skill building interventions and other supportive interventions, the Peer Specialist is able to assist fellow consumers in effectively utilizing the resources, managing symptoms of their illness, and developing daily living skills to gain greater independence, employment, and other meaningful activities. This position is responsible to the Division Manager and/or Coordinator of the assigned program. The Peer Specialist reports to the Team Leader of the assigned program.
Revenue Cycle Management Manager
The Revenue Cycle Management Manager oversees the daily operations of the billing and reimbursement team including; claims submissions, patient accounting activities, open claims, monthly reporting.
Clinical Quality and Assurance Manager
The Clinical Quality and Assurance Manager is responsible for planning and implementing the performance improvement program to meet the needs of the agency.
